Moving forward with Composr

Hello all,

I am an ocPortal newbie and am just getting started with customizing my site. I was looking at a way to create/use a submit form for my site.
I found the custom template for forum submissions, but it's just a "text template" that people can edit and erase before they post it. I'm looking at something more along the lines of a "required fields" submit form.
More than just a "site sign-up", this is specifically an application for membership to an online guild (it's a "clan" community gaming site).

Ideally, I'd like to have:
  • Required fields
  • Optional fields
  • Text input fields - single word and also free text
  • Drop-down select boxes
  • Check boxes
  • Posted to forum*
*The posting to forum part is optional. If this needs to be a menu item -> page, then that is absolutely fine as well.

Is there a way to do this already built into ocPortal?
is there a plugin that will do this already?
Disclaimer: I am not really a programmer, so if one of the solutions is to create a submit form and put that in a page, that's fine, but I would need to find some existing HTML code to hack apart and customize for that. I'm trying to avoid that if possible.

Create a catalogue for people to submit to, or create an HTML form and post it to data/form_to_email.php (that'll accept arbitrary form data and send it to the staff).

I did find some information on catalogues and configuring them, but not how to add them.  Is it possible I disabled them during install?  If so, what/where do I look to re-enable it?

Ah ha.  It was disabled at install.
I've enabled it and created a catalogue.  Now I need to figure out how to use it.  I'll have to keep looking at the docs.
I thought I added it to a page, but all I see is "There are no entries at this time".

Okay, I give.  :(
Is there a tutorial or link somewhere that explains how to display this catalogue as a submit form?  I created a bunch of fields in this catalogue, but I can't figure out how to turn it into a submit form.  I am feeling very dense right now.
