possbile? tips and advice?

I am wondering if this is a possibility. I am thinking it might be having seen some things brought up, but to be quite honest here I am not quite sure on where to begin to even attempt this, so any advice, tips, pointers, etc… would be greatly appreciated.

What I would like to do is to take the character sheet from my book and use it as an online form using the catalog system that ocPortal provides, so that members can fill it out online and have it stored online for use with my online forum RP, Legends of Nor'Ova: Saga of Ablution.

The questions I have is how excatly would I make that a reality lol. A big question no? For example I saw in one thread here about using something like a form tag, but how would I make it so that not only is the form tags connected to the catalog/database but also allowing comcode support so that people can use all those spiffy formating tools?

I don't know if it will help any, or if anyone can give me any advice on this matter, but just in case, here is the character sheet from my book. It's in pdf format, so you'll need a pdf reader to view it. Luckily there are many out there that are free lol.

Thanks in advance for any information, how-to's, tips, etc.
