Hi,

I can't speak for what inputs you've tried but from the screenshot and the error it is easy to put this down to incorrect database login details (it looks like you're trying to rely on defaults, but these details are tailored and specific, not something that could be guessed at or defaulted). You're likely installing on an internet web server rather than you're own computer, in which case the username would not be 'root' (this is the administrative username for a whole server).
The exact details to use depend on your web host, but in the process of setting up a database on the server, the username and password and database name for that would have either been chosen by yourself, partially chosen, or would have been chosen for you - either way, they'd be apparent at some stage of that process. You need to input these specific details so that ocPortal can connect to an existing database.

No, the database details are specific to an individually created database, and not derived from anything else. This database needs setting up on the web server. If you're not sure how to do this, you'll need to ask your web host or refer to their documentation.
