If it does exist, perhaps uploading a fresh copy to overwrite the current one might resolve the issue. It is always possible that file got corrupted …!! P.S. … don't forget to flush your caches!
Take my advice. I'm not using it!

After chaning the position of string REPORT_POST in ocf.ini (i.e. to the top or bottom), the issue seems to have gone away. |

Above my pay grade, but congratulations on finding a 'fix'. Something else I noticed on your page … … haven't had time to 'pursue' it from this end, but perhaps you will be able to get directly to the problem, knowing what code you've got, and where!
Take my advice. I'm not using it!

However, I still don't understand why moving string REPORT_POST to somewhere within ocf.ini (doesn't matter to what position) seems to "fix" the issue. I VI'ed it in binary mode to check for any special characters, but no issue. Although I have no clue about the root cause, I am happy that fixed my problem |

It might not have been the change itself… Changing the file probably caused the smart-decacher to refresh the cache.
